Follow-ups to #54/#56.

Live refresh: the tree, Changes and open files refresh after Claude's turn ends or on ↻. A watcher (inotifywait on the open folders, or polling mtimes of the expanded folders every few seconds while the pane is open) would show new and changed files while Claude is still working.
Replace in files: a Replace box in the Search view with a preview of each change, applied per file through --fs-write with the sha check, so nothing Claude changed meanwhile is overwritten.
